Transformations are temporary changes to the player character that can be caused by interaction with enemies and objects.
Ball
First appearance: Pizzascape
Peppino or The Noise will roll like a ball, smashing everything in his way. While in this form, they can't do anything except jump, only gaining back control after contacting a wall.

Knight
First appearance: Pizzascape
Peppino, The Noise or Snick wear knight armor and are super heavy, causing enemies to be crushed when jumping on them. When touching a slope or a staircase, they will slide down it at high speeds, defeating any enemies and breaking any blocks (other than transformation-specific blocks) in the way. Hitting a wall after sliding down a slope or staircase will get rid of the transformation. Hitting outlets also gets rid of the transformation.

Bomb
First appearance: The Ancient Cheese
Peppino, The Noise or Snick will hold a bomb and start uncontrollably running around until the bomb explodes, which ends the transformation. In this form, the only actions that can be done are jumping and turning around. This transformation is one of the only ways to break bomb blocks.

Firebutt (Peppino) / Firecape (The Noise) / Firefur (Snick)
First appearance: Bloodsauce Dungeon
Peppino, The Noise, or Snick will jump high up in the air and yell as his butt (Peppino), cape (Noise), or fur (Snick) burns until he hits ground.
-
Firebutt Peppino.
-
Peppino butt sliding after landing.
-
Peppino's butt being exposed from the fire. This animation is now unused in the newer builds, likely due to it being time-consuming.
-
Firefur Snick.
Weenie Mount
First appearance: Dragon Lair
Peppino or The Noise mounts a giant sausage and is allowed to traverse across dangerous terrain while moving non-stop at high speeds, plowing through any enemies in the way. In this state, he can't get hurt. However, if he hits a wall he'll be knocked off the sausage, ending the transformation.
Firemouth
First appearance: Desert level
Peppino will spout fire out of his mouth, running around uncontrollably. In this state, he can shoot fire that can explode TNT and defeat enemies. Besides being able to shoot fire, it works very similarly to the Bomb transformation. The transformation can only end by reaching a free milk stand.
-
Firemouth Peppino.
Ghost
First appearance:Cowboy/graveyard level
Peppino becomes a ghost and can fly by jumping repeatedly and go through cheese graters while also being invincible, but can't use any sort of attack. Touching a tombstone will end the transformation. Tombstones can be grabbed by Peppino and placed in a different location before entering the transformation.

-
Ghost Peppino turning back to normal.
Mort
First appearance: Farm level
Mort the Chicken perches on top of Peppino's head. With Mort on his head, Peppino can have him smack his head in front of him and use him to descend slower in the air. This transformation is also the only way to break Mort cubes. Getting hit a certain amount of times will knock Mort off of Peppino's head, ending the transformation.

Barrel
First appearance: Beach level
Peppino gets stuck in a barrel and moves slower, but can roll quickly when slipping on a banana peel to defeat enemies and move faster, and traverse across water by floating above it. Hitting a wall after slipping on a banana peel will cause the barrel to break, which ends the transformation.

Cheeseball
First appearance: Sewer level
Peppino or The Noise gets stuck in a cheeseball and will roll forwards until hitting a wall, which ends the transformation. In this form, their jumps are much smaller than normal. This transformation is the only way to break cheeseball blocks.
Boxed
First appearance: Factory level
Peppino gets trapped in a pizza box. In this state, he is able to go through small gaps at a faster speed than crawling, however he has no way to attack. Getting hurt or getting grabbed by a Grabbie Hand ends the transformation.
-
Boxxed Peppino.
Sticky Cheese
First appearance: Factory level
Peppino gets covered in sticky cheese with pepperoni covering his eyes and lettuce covering his eyebrows and mustache. In this form, he is able to stick to conveyor belts and do a wall jump off them. Touching an outlet ends the transformation.

Scrapped Transformations, all except for the "hungry" transformation can be seen in the "ending" room of demo 1
-
Cape Peppino. This would've let Peppino fly and destroy metal blocks. The design is weirdly similar to the original design for the Noise, it might've been the inspiration for his design after the transformation was scrapped.
-
Meteor Peppino. Once launched by a strong enemy, he would fly forward, having the power to break metal blocks and bounce off any slope he touches.
-
Big Peppino.
-
Spinning Peppino.
-
Drunk Peppino. Very clearly a reference to the "Drunk" Wario transformation in Wario Land 2 and 3.
-
Caffeinated Peppino.
-
Dash-bite/Hungry Peppino. Once he would get in contact with an aroma, he would make a beeline to where it would be coming from.
-
Confused/Flustered Peppino. Activated when in contact with a seductive pizza lady enemy.
-
Sick Peppino.
