The first Toppin is in the second room of the stage, viewable in the open. After passing over both pools of lava, the player can run up the wall into the boxed-in area where the Toppin is.

Bloodsauce Dungeon is the fourth level in Pizza Tower and the fourth level of the Tower Lobby.
The level is an almost entirely vertical, bone-filled dungeon, with different layers the deeper you go.
The level's titlecard shows Peppino and Gustavo, the former in a viking-like outfit while holding an axe while the latter is wearing more typical knight armor while holding a torch, in a dimly lit section of the dungeon, stalked by a menacing-looking Pencer and Cheeseslime inside of a giant skull.

In the room directly after the Mushroom Toppin, before the player descends downward to the next room, the second Toppin can be found on the left side of the bottom of the room, only guarded by a pizza cutter below it.
The third Toppin is in the room directly after the previous Toppin, located on the player's path forward out in the open.
The fourth Toppin is impossible to pass by, as it must be walked through to continue in the gray room at the bottom of the stage.
In the same room as the Sausage Toppin and third secret, the player must maintain Mach 2 to break through a Metal Block to reach the last Toppin. It is located in sight of the player, just behind a Pencer.

Secrets
In the third room of the stage, the player must maintain momentum in the area with two pizza cutter hazards to keep Mach 2 and break a block that holds a visible secret eye behind it. In this secret, the player simply runs forward and occasionally dives through breakable blocks.
In the first pitch-black room, near the exit from the room, there's a gap in the ceiling. The player needs to super jump into it to reach the secret, which is visible from higher up in the room. When the secret is complete, a one-way block can be broken as a shortcut to the higher area of the room. In this secret, the player must maintain momentum while hopping over lava pools, then break some blocks to reach the exit eye by jumping into a lava pool and bouncing up into it. Because the player uses a lava pool to exit, it is not recommended to play this stage when trying to complete the Chef Task "Very Very Hot Sauce".
Once the player has collected the Sausage Toppin, the player must run up the right of the wall above where the Toppin was. This secret is a perilous chamber full of pizza cutters, and must be traversed slowly and carefully to avoid damage.

Trivia
- The music track heard whenever the player is near Pillar John, "Meatophobia", was originally only planned to play at the end of Bloodsauce Dungeon during its final colder rooms.
- Despite having final name in SAGE 2019 Demo, it was temporarily called Dungeon in PT_Eggplantv15 build.
- Bloodsauce Dungeon has a harder, yet cheerier version in the form of Strongcold. Strongcold was first introduced in Peppino's Xmas Break and was later removed in PT_Eggplantv15.
- Part of the melody of the track that plays here is taken from the Beastie Boys' "Intergalactic", which in turn sampled Les Baxter's "Prelude in C# Minor".
- This is the only level in the game which contains Flying Anchovies.
