John Gutter is the first level in Pizza Tower, and is the first level of the Tower Lobby. This level serves as a It also serves as a testing ground for Peppino, while including many of obstacles for the player to test.

Collectibles
Gerome's door is located in the seventh room. The easiest way to access this door is during Pizza Time, where the player has to run into a Metal Block and superjump to the top.
Secrets
In the room to the left of the area with the Cheese Toppin, part of the hanging ceiling of the level is discolored and has an eyeball scribbled on it, near where the player enters the room. The player must super jump into this block to break it, entering the secret room.
This secret room consists of small solid platforms with small and large toppings sprinkled on them, connected by strings of semisolid platforms. The player must turn very quickly on these platforms to retain a constant dash, though it is not necessary to complete the secret.
In the room with platforms that leads to Pillar John's room, the rightmost area on the floor has a spot with a white eye marked on the floor. Ground pounding this spot breaks the ground to reveal a secret.
This secret stage is a short vertical climb where the player must use the mach dash to scale the walls, automatically grabbing the large toppings on top. The player will have to hop to change direction once, as to not run off the edge of the map.
After Pizza Time has begun, the player will enter a room in which they must progress downwards while fighting enemies. The player needs to run into a wall with cracks on it at Mach 2, which is located on the segment directly before the player can descend down into the warp box. Mach 2 can be reached by running at the wall directly from the opposite wall.
This secret requires the player to mach dash through steel blocks while retaining momentum by wall jumping.

- "Primate Rage" requires player to get a combo of 99 in this level. This means every single enemy must killed in the level, including Lap 2.
Trivia
- As the selection of playable levels expanded over the course of game's development, John Gutter became the third to be the introductory level of a demo release. The first was Oregano Desert and the second was Pizzascape.
- The long Pillar John seen in the first few rooms changes appearance during Pizza Time. It gains a more angry look and opens its eyes, revealing yellow-ring shaped pupils, like those of the regular Pillar John.
- Since the assets of the level were designed before Pillar John's current design, the Johns are noticeably rounder and have brow ridges instead of eyebrows.
- John Gutter used to be a tutorial level.
- After beating The Noise, the level was originally supposed to permanently transform into Noise Gutter, all of the Forknights replaced by by Noiseys. The level music would also be replaced with "The Noise's Jam Packed Radical Anthem".
- This level was at first titled Entrance in the December Patreon Build before getting its final name.
- In this level, Pizza Tower Guy is located in the Pillar John room, near John himself.
